Job Responsibilities
Case managers coordinate the treatment of behavioral health conditions (mental and substance use) and practice concerns of healthcare professionals by providing comprehensive care management and coordination to assist safe practice.
Qualifications
Preferred: LPN or RN
Also considered: Other qualified candidates with relevant experience
Must have: Excellent oral & written communication
