Job Summary
The MAPS Coordinator works directly with individuals, families, and support teams to coordinate services, create person-centered plans, and ensure that individuals’ needs and wishes are supported through advocacy and collaboration. The role combines case management, family engagement, and resource coordination to promote independence and inclusion.
Work Schedule

Flexible schedule with the option to work from home.

Full-time position (up to 37.5 hours per week).

Start and end times may vary depending on meetings, visits, and trainings.
Job Responsibilities
-
Coordinate intake and referral processes for MAPS participants.
-
Lead and facilitate Person-Centered Mapping and Planning meetings.
-
Develop and monitor Person-Centered Plans based on individual goals and outcomes.

Educate participants and families on topics including:
-
Home safety, personal hygiene, and health management.
-
Financial management and public benefit programs (e.g., Social Security).

Employment readiness, community participation, and independent living skills.

Conduct assessments, monitor progress, and document services.

Collaborate with internal staff and community partners to ensure consistent, person-centered support.

Maintain compliance with The Arc and DIDD policies and procedures.
Qualifications
Education/Experience (one of the following):
-
Bachelor’s degree in a human services field; or
-
Bachelor’s degree in another field plus one (1) year of relevant experience; or
-
Associate degree plus two (2) years of relevant experience; or
-
High school diploma/GED plus four (4) years of relevant experience.

Knowledge of state and federal regulations related to disability services.

Excellent communication, organization, and critical-thinking skills.
-
Ability to work independently and collaboratively as part of a team.

Proficiency with Microsoft Word and basic computer skills required.

Familiarity with Tennessee’s Department of Intellectual and Developmental Disabilities (DIDD) and MAPS program preferred.
