Job Summary
As a clinic or social services case manager at Nashville CARES, you will develop relationships with clients and support them through many stages of life while learning and growing in your own career. The goal of case management at Nashville CARES is to reduce barriers to accessing healthcare.

Job Responsibilities
What You’ll Do
- Provide compassionate and respectful care and comprehensive case management services to help clinic clients address the complex challenges associated with living with HIV
- Meet regularly with clients in the office, in clients’ homes, and in the community
- Conduct biopsychosocial assessments on all clients on a semiannual basis
- Assist clients with navigation of medical, social, community, financial, and other needed services
- Ensure clients have access to health benefits and knowledge of how to utilize them
- Assist clients in developing realistic action steps to achieve their goals
- Provide support services and referrals to clients who may be socially isolated
- Advocate for clients within the agency and in external settings, such as medical providers and other agencies they may be engaged with in the community
- Complete all documentation in an appropriate manner and in a reasonable timeframe according to agency standards to ensure clients do not encounter barriers to service
- Practice appropriate boundaries with clients and with work to ensure you are able to provide excellent services to clients while preserving self-care

- A fluent Spanish-speaker who is passionate about their clients and their work, but who also understands the importance of good boundaries
- A clear communicator who can explain complex ideas in ways that are easy to understand
- A problem solver who can see both details and the bigger picture and find balance between the two
- Someone who can independently research solutions and resources while also knowing when to ask for help
- A team player who also works well independently
- A person who finds value in doing good work
- Someone who is organized and can keep track of a variety of clients and their needs
- A person with excellent documentation skills
Qualifications
Minimum Requirements:
- Master’s degree (preference for Master’s in Social Work or other Human Services) – must be able to be verified
- Fluency in Spanish and English
- Minimum of two years’ experience in community based social or health care services
- Proficiency with a variety of technology, including Microsoft Office software (Word, Excel, Outlook), electronic health records (training provided), and general Windows PC and cell phone technology
- Valid Driver’s License, vehicle, and automobile liability insurance to facilitate local travel required for client/agency-related work
