Job Summary

The Agency LPN provides essential medical oversight and coordination to support individuals with intellectual and developmental disabilities (IDD) in community-based settings. This part-time role ensures the health and safety of service recipients through medication administration support, health monitoring, documentation review, and staff/provider communication. Familiarity with Tennessee’s DIDD and DDA guidelines is preferred.

Job Responsibilities

Medical Coordination & Oversight:

· Review and track medical records, annual physicals, consents, and specialist appointments.

· Monitor MARs (Medication Administration Records) for accuracy and completion.

· Conduct monthly medication audits and health status checks for assigned service recipients.

· Provide oversight of medication storage and administration compliance in family model homes or CLS programs.

Documentation & Compliance:

· Ensure health-related documents are complete, current, and in compliance with DIDD and DDA regulations.

· Maintain timely communication with physicians, pharmacies, and clinical team members.

· Collaborate with case managers and program staff to respond to medical-related reportable events (REMs).

Support & Coverage:

· Fill in for the Agency RN during periods of absence, ensuring continuity of care, documentation oversight, and nursing responsibilities.

· May be required to provide on-call support during evenings and weekends on a rotating basis.

Quality Monitoring:

· Participate in internal quality monitoring reviews and respond to follow-ups related to medical care.

· Identify trends or concerns in health data and propose corrective actions as needed.

Qualifications

  • Current LPN license in good standing with the State of Tennessee.
  • · Minimum 2 years of nursing experience, preferably with individuals with developmental disabilities or in a home/community-based setting.
  • · Understanding of DDA/DIDD policies and documentation requirements strongly preferred.
  • · Strong organizational and time-management skills.
  • · Proficiency in electronic health records and/or Microsoft Office Suite.
  • · Ability to work independently and communicate effectively with a multidisciplinary team.
