Interfaith Dental is a volunteer-driven nonprofit agency that helps vulnerable populations achieve and sustain a healthy smile. Interfaith Dental’s mission is to create a healthier community by providing transformational oral health care for those experiencing poverty. Interfaith Dental now serves vulnerable populations in two clinic locations within a ten-county region of Middle Tennessee. Interfaith Dental focuses on life transformation through dentistry offering both access to oral health services as well as education, motivation, and inspiration.
Job Responsibilities
The role you will play
The Community Dental Health Coordinator serves as a liaison between patients, physicians, dentists, community health centers, senior centers, and other social service agencies to facilitate access to care and enhance the quality-of-service delivery. The primary goal of the SMILE ON 65+ program is to educate, motivate, and guide older adults into establishing dental homes. This position reports directly to the SMILE ON 65+ CDHC/CHW Director.
Duties/Responsibilities:
- Develop and facilitate community-based and individual oral health prevention programs that promote oral health and disease prevention education in a manner that matches participants or community.
- Develop and facilitate community-based and individual oral health prevention programs that promote oral health and disease prevention in a manner appropriate to the population served.
- Provide oral health education, screenings, triage, and case management to support improved oral health outcomes.
- Assist dental providers with direct patient care when needed.
- Implement strategies to empower older adults and service providers to advocate for improved access to dental care.
- Navigate the dental care system to guide and support individuals in accessing appropriate services and achieving better oral health.
- Maintain accurate and timely documentation of demographic information, clinical interactions, and navigation notes within the program’s software system.
- Prepare written reports summarizing patient dental health status and identifying barriers to improved oral health.
- Participate in regular meetings with SMILE ON 65+ and Interfaith Dental teams to discuss provider updates and partner agency collaboration.
- Serve as an ambassador when working with SHIP navigators, Area Agencies on Aging and Disability (AAADs), nonprofits, public health departments, and senior medical providers.
- Provide administrative support to community partners, Interfaith Dental, and the SMILE ON 65+ program.
- Perform other duties as assigned.
Qualifications
What you will bring:
- Registered Dental Assistant (RDA) or Registered Dental Hygienist (RDH).
- Completion or willingness to complete a Community Dental Health Coordinator certificate from an ADA-approved school.
- Registered Dental Assistant (RDA) or Registered Dental Hygienist (RDH) required.
- Completion of, or willingness to complete, a Community Dental Health Coordinator (CDHC) certificate from an ADA-approved institution.
- Completion of, or willingness to complete, SHIP certification.
- Completion of, or willingness to complete, IMPaCT Community Health Worker (CWH) training.
- Ability and means to travel within the state as needed.
- Capable of working independently as well as collaboratively within a team environment.
- Proficiency in Microsoft Office Suite, including Excel.
- Strong time management skills with the ability to prioritize tasks and manage multiple responsibilities effectively.
- Experience in outreach, education, and problem-solving.
- Dental office background and/or experience preferred.
- Excellent written and verbal communication skills.
- Demonstrated ability to work comfortably and respectfully with individuals from diverse cultures, backgrounds, lifestyles, and experiences.
