Job Summary

The Family Liaison serves as the primary point of contact for families seeking support from the Jordan Thomas Foundation. This role functions similarly to a case manager and is responsible for guiding families through the initial application and approval process, coordinating ongoing support services, and maintaining long-term relationships as families continue their journey with JTF. The Family Liaison provides compassionate assistance, ensures clear communication, and helps remove barriers so families can focus on their child’s well-being and care.

Job Responsibilities

Family Support & Case Management

  • Serve as the first point of contact for families inquiring about JTF services.
  • Assist families with the application, documentation, and approval process.
  • Conduct intake interviews to understand family needs, background, and goals.
  • Provide clear, timely communication regarding status updates, next steps, and program requirements.

Ongoing Family Engagement

  • Maintain supportive, long-term relationships with JTF families throughout their time in the program.
  • Coordinate with medical providers, prosthetic teams, and other partners to ensure seamless care.
  • Help families navigate challenges by connecting them with resources, education, and advocacy support.
  • Facilitate follow-up communications and check-ins with families to track progress and needs.

Program Coordination & Administration

  • Document all interactions, updates, and case information in JTF’s database.
  • Collaborate with internal staff to ensure alignment of services and organizational goals.
  • Contribute insights to improve processes, family experience, and support initiatives.
  • Assist in planning family-related events, workshops, or community activities.

Communication & Relationship Management

  • Represent JTF’s mission with empathy, professionalism, and care.
  • Provide families with education on available services, prosthetic care, and JTF expectations.
  • Act as a bridge between families and the Foundation to ensure their voices and needs are heard.

Qualifications

  • Bachelor’s degree in Social Work, Human Services, Psychology, Child Development, or related field (or equivalent experience) preferred.
  • Experience in case management, family support services, nonprofit work, or a related role.
  • Strong communication and interpersonal skills, with the ability to build trust and rapport.
  • Ability to manage multiple cases, prioritize tasks, and maintain accurate records.
  • Compassionate, patient, and adaptable approach to working with diverse families.
  • Experience working with children with medical needs is a plus.
  • Proficiency in Microsoft Office and comfort with case management or CRM systems.
