Job Summary
This is an open call for proposals to build a full, comprehensive platform for data collection, management, and storage for community health workers (CHWs). This platform will be utilized by CHWs to capture patient data, manage caseloads, interact with patients, conduct proactive outreach leveraging predictive analytics, and standardize care workflows. This platform is meant to lift the administrative burden off of CHWs to enable them to spend more time conducting patient outreach and managing patient caseloads. CHWs provide outreach to patients in need to address self-reported social determinants of health and connect them to resources and nonprofits to remove these barriers.
The selected vendor will be responsible for the end-to-end design, development, testing, deployment, and support of the platform.
Job Responsibilities
Functional Requirements
- Streamline documentation using mobile-friendly data capture
- Use predictive analytics to surface at-risk patients and prioritize workflow
- Standardize care workflows using intervention templates and smart logic
- Use AI to assist with patient communications
Non-Functional Requirements
- HIPAA Compliant
- Secure Cloud environment
- Ability to scale system-wide
Timeline
This project utilizes grant funding and must be completed by May 31, 2026.
